Blood donor questionnaire and declaration
Blood donation within Australia
In Australia, the collection and supply of blood and blood products is managed by Australian Red Cross Lifeblood (Lifeblood), under a deed of agreement with the National Blood Authority.
To ensure the safety of blood donors and the Australian blood supply, Lifeblood uses a blood donor questionnaire to assess the suitability of individuals to undergo blood donation and to screen for potential risks that could affect the quality and safety of donated blood products. Donations are also tested for evidence of infectious diseases including hepatitis B, hepatitis C and HIV.
In Western Australia, the donation and therapeutic use of blood and blood products is regulated under the Blood and Tissue (Transmissible Diseases) Regulations 1985 (WA). As part of the blood donor questionnaire, donors are required to make a declaration that the information they provide is given honestly and to the best of their knowledge. Providing false or misleading information on the blood donor questionnaire and declaration may have criminal consequences. In Western Australia (WA), the blood donor questionnaire must be approved by the Chief Health Officer, WA Department of Health.
The Lifeblood blood donor questionnaire and declaration form is regularly reviewed and updated to ensure it is based on the most up to date scientific evidence. The current Lifeblood blood donor questionnaire is available below:
