Health Navigator
- Health Navigator is a free service.
- It is available for people living in the Wheatbelt, South West and Great Southern regions and Bunbury.
- The service helps people manage their chronic health condition and get their health on track.
Health Navigator is available for people with one or more of the following health conditions:
People at risk of developing diabetes or heart disease – including people with excess abdominal weight, raised cholesterol, raised blood glucose or high blood pressure – can also be referred to the Health Navigator Service.
Health Navigator is provided by the WA Country Health Service and Silver Chain as part of the Southern Inland Health Initiative; a half billion dollar investment in health that has transformed access to medical and emergency care across a large part of inland WA, made possible by the State Government's Royalties for Regions program. This service is supported by funding from WA Primary Health Alliance via the Australian Government through the Primary Health Networks program.
How does Health Navigator work?
Health Navigator coordinators work with you to ‘navigate’ the health system and to develop personal goals to improve your health.
Health Navigator coordinators work with your GP and other health care providers so that you receive coordinated care. This means everyone works together to help you achieve your health goals. The service is conducted over the phone or via video conference, which means you can access support and advice from the comfort of your own home.
Many Health Navigator clients are now enjoying activities, interests and family life with the support of the Health Navigator team.

How can Health Navigator help me?
You can contact Health Navigator yourself if you have one of the listed health conditions. Alternatively, your doctor or another health professional, such as a GP, diabetes educator or a nurse practitioner, may refer you to Health Navigator.
Health Navigator can help if:
- you feel that you need more regular support and advice to achieve your health goals
- you want help to overcome any barriers stopping you from accessing the health services you need.
There are many ways Health Navigator may be able to help you, including arranging transport to appointments or arranging help with meal preparation and shopping to ensure you are eating well.
Health Navigator coordinators may:
- talk to you on over the phone about your health and any health goals you have
- develop a ‘My Plan’ with you to work out the steps you need to take to improve your health. The ‘My Plan’ is useful to take along to all your health appointments.
- provide information and advice about how to access the services you need
- arrange any appointments or referrals you need
- make regular contact to see how you are progressing, and to offer support and advice if you are finding it difficult to achieve your My Plan.
Client Information Sharing System
The Client Information Sharing System (CISS) is part of the Health Navigator service. It's a secure electronic record that allows your health care providers to easily view and share health information about you, such as a stay in hospital, visit to your doctor or results of blood tests. CISS is only a summary of your health information, and not a complete record.
