Advance Care Planning Week, 16 to 22 March, is a great time to start your advance care planning journey.
It’s a voluntary, flexible process – you can involve as many or as few people as you wish, and update your decisions if your preferences change. It involves 4 key parts: think, talk, write, and share.
There are a range of documents to help you record your preferences. Each document serves a different purpose, and you can choose which ones are most relevant to your situation.
These documents are a formal way to ensure your wishes are clearly recorded and easy for others to follow. You can share these with family, friends, carers, and health professionals.
Everyone should consider advance care planning, regardless of age or health. It’s especially important to people who are over 75, are at risk of losing competence, or have an advanced chronic illness or a life-limiting illness.
